All new patients are required to be up to date on vaccines in accordance with the CDC vaccine schedule. Patients with small delays will be reviewed on a case-by-case basis.

Before scheduling your first appointment
Please email the following to info@mindfulpeds.com with NEW REGISTRATION in the subject line:
Registration Packet (must be filled entirely)
Copy of parent/guardian’s ID
Copy of insurance card front and back
Copy of immunization record
After we have received registration completely, first appointment can be made.
